OMG, OMG, what is happening to Sapphire.
Sapphire has been re-closed by the New York City Health Department.
Yes, Sapphire located at 1845 Broadway (near Lincoln Center) has been re-closed by the NYC Health Department after a follow on inspection unearthed new hygiene issues on April 15, 2011.
The NYC Health Dept. had previously closed Sapphire following the April 13, 2011 inspection in which the restaurant notched up a whopping 67 Violation Points.
While re-closing Sapphire on April 15, 2011, the NYC health inspection cited among other things toxic chemical is improperly labeled, stored or used such that food contamination may occur, and live roaches present in facility’s food and/or non-food areas.
Besides toxic chemical and live roaches issues, the NYC Health Department’s latest inspection report also cited evidence of mice or live mice present in facility’s food and/or non-food areas, and food not protected from potential source of contamination during storage, preparation, transportation, display or service.

Sapphire received 27 Violation Points in its April 15, 2011 NYC health inspection.
Here is the full list of Sanitary Violations observed at Sapphire during the April 15, 2011 inspection: (Critical Violations in red color):
1) Toxic chemical improperly labeled, stored or used such that food contamination may occur.
2) Evidence of mice or live mice present in facility’s food and/or non-food areas.
3) Live roaches present in facility’s food and/or non-food areas.
4) Food not protected from potential source of contamination during storage, preparation, transportation, display or service.
5) Facility not vermin proof. Harborage or conditions conducive to attracting vermin to the premises and/or allowing vermin to exist.
