Unhygienic Sapphire Re-Closed by NYC Health Dept.;
Toxic Chemical, Live Roaches Issues Surface
Sapphire, a crappy NYC Indian restaurant that we’ve dined in, located at 1845 Broadway (near Lincoln Center) has been closed by the New York City Health Department following a health inspection on April 13, 2011.
Sapphire received a whopping 67 Violation Points in the April 13, 2011 health inspection.
According to the April 13, 2011 NYC health inspection report, Sapphire has evidence of mice or live mice present in facility’s food and/or non-food areas and raw, cooked or prepared food is adulterated, contaminated, cross-contaminated, or not discarded in accordance with HACCP plan.

Also, food at Sapphire is not protected from potential source of contamination during storage, preparation, transportation, display or service.
Besides the hygiene related sanitary violations, Sapphire notched up a weird violation.

Here is the full list of Sanitary Violations observed at Sapphire during the April 13, 2011 inspection: (Critical Violations in red color):
1) Cold food item held above 41º F (smoked fish and reduced oxygen packaged foods above 38 ºF) except during necessary preparation.
2) Raw, cooked or prepared food is adulterated, contaminated, cross-contaminated, or not discarded in accordance with HACCP plan.
3) Evidence of mice or live mice present in facility’s food and/or non-food areas.
4) Food not protected from potential source of contamination during storage, preparation, transportation, display or service.
5) Facility not vermin proof. Harborage or conditions conducive to attracting vermin to the premises and/or allowing vermin to exist.
6) Non-food contact surface improperly constructed. Unacceptable material used. Non-food contact surface or equipment improperly maintained and/or not properly sealed, raised, spaced or movable to allow accessibility for cleaning on all sides, above and underneath the unit.

Folks, Sapphire is a serial offender when it comes to poor hygiene practices and notched up more than 40 violation points in the last two health inspections performed on December 21, 2010 and November 15, 2010.
